Kindergarten Round Up 2025-2026 Health Requirements

View the following details regarding health requirements for scholars who will be starting kindergarten for the year 2025-2026.

Kindergarten Entry Health Requirements 2025-2026

Health Assessments

  • Physical Examination (with lead screening and developmental screening) – performed by a licensed physician, advanced practice nurse, or physician assistant

    • Must use the State of Illinois Certificate of Child Health Examination form

    • Must be completed within 12 months of August 5, 2025 

  • Vision Examination – performed by a licensed optometrist or ophthalmologist

    • Must be completed within 12 months of August 5, 2025

  • Dental Examination – performed by a licensed dentist

    • Must be completed within 18 months of May 15, 2025


Immunization Record

Proof of the following immunizations must be submitted:

  • Diphtheria, Tetanus, Pertussis (DTaP, DTP)

  • Polio

  • Measles, Mumps, Rubella (MMR)

  • Varicella (chicken pox)

  • Hepatitis B

Religious Exemption

All religious exemptions to immunizations must be filed using the Illinois Certificate of Religious Exemption to Required Immunizations and/or Examinations form. No other type of documentation will be accepted.


Please turn in all forms on or before the first day of school to prevent missed school time for appointments. We highly recommend scheduling your child's medical appointments early, as summer appointments fill quickly.


Health Conditions

If your child has a food allergy, asthma, seizure disorder, diabetes, or requires a medical procedure during the school day, please download the appropriate form(s) from the district website and have the child’s healthcare provider complete them. This is essential for the proper care of your child during the school day.

Medication

If your child requires medication at school, a Medication Authorization form must be completed for each medication. The form must be completed and signed by both the parent/guardian and the child’s healthcare provider. No medication will be given without a completed Medication Authorization form on file.
